J'ai eu le meme pb, j'ai la soluce mais dsl c'est un copier/coller d'un readme.txt en anglais.
Si tu suis toutes les indications apres ca marche sous UT2003.
Note seuleument qu'il faut absolument les drivers 9.75 de Logitech pour que cette manip fonctionne.

Many people have been having more problems with the Logitech 9.75 drivers in reference to games such as Unreal Tournament 2003. Especially in regards to button functionality on the new MX 500 and 700. While the 9.73's allowed limited functionality inside the game with the thumb buttons. The 9.75 disabled this in dome games (most notably ut2k3) in order to allow wheel movement. Personally, I do not use the mouse wheel as much, I would have gone back to the 9.73's. However, the 9.75 have increased mouse sensitivity without resorting to any acceleration. Very important for precise shooting.
I have discovered a method that will not only restore the function of the thumb buttons, but enable all the rest (7 total). Although, this comes at a price as you will loose the quick program switch function (alt-tab), and universal scroll in Windows. I also cannot guarantee any better or worse results in other game programs. so THIS IS A USE AT YOUR OWN RISK!
This requires changing/adding two functions in your registry. If your afraid to do this yourself, double click on the, "UT2k3_MX_enable.reg" file and click, "Yes" to the registry editor dialog box. Then jump down to #6 below. Else....
YOU MUST HAVE ALREADY INSTALLED MOUSEWARE 9.75 FOR THE BELOW TO WORK!

1. Open the start menu and click on "run"
2. Type in "regedit"
3. Inside regedit follow the this path to get to the area you need:
HKEY_LOCAL_MACHINE\SOFTWARE\Logitech\MouseWare\CurrentVersion\GamingCompatibility
Inside here find the following string: UnrealTournament3
You will notice that this is Unreal Tournaments executable name. You could delete this whole string. But to be safe without effecting other programs.. Change the Value data to: UT20045.exe. By doing this the 9.75 drivers will no longer disable EM_EXEC.EXE when UT2k3 starts up(the Logitech process that controls the added button functionality).

Now you would think this would be enough. However, if you try to use the mouse now the middle/scroll button itself no longer functions while scrolling itself does. No matter if you use the Logitech drivers to change the function of that button. Since personally I use this button for forward. This was unacceptable.
To overcome this, I accidently discovered the below:
4. Slightly below this folder is the: Technical
HKEY_LOCAL_MACHINE\SOFTWARE\Logitech\MouseWare\CurrentVersion\Technical
Inside here right click and choose to create a new string value. Call this new value: MouseHookDllEnable
Click on this new string and give it a value of 1 .
This was initially part of a Logitech suggestion to fix scrolling functionality in older mouseware. However, in that case you set the values to 0 (disabled). In this case we set it enabled (1)
5. You must now reboot the machines for the above to register.
6. Upon Windows entry, go into the Logitech Mouse Control Panel. Here is the part that may turn some of you off. As you loose the quick program switcher and universal scroll in other programs, overall though, the mouse will function very much the same.
Here is the setup I used on my mx500:
1. Click Select
2. Forward
3. Back
4. [Page Up]
5. Context/Alternate Select
6. Middle Button
7. [Page Down]
8. Up to you, but you must use a keyboard emulation. In my case I set it to [F1] so I can have easy access to score/pings.
NOTE Limitations: 2,3,4,7 and 8 will only work with commands that are "immediate". The mouse hardware itself seems to be limited to an on/off state with these buttons. So any type of movement command will not work as the buttons only register for a brief second they are clicked.

I also will note I used a Windows XP machine with Nvidia 41.09 drivers. Your luck may vary on different software/hardware.
